Patient Forms
If you are a new or existing patient, please visit our patient forms page to download a copy of our Notice of Privacy Practices. As a courtesy, we email all new patient forms electronically to you before your initial orthodontic consultation. Though we strongly encourage you to complete the electronic forms prior to your first appointment, we also allow forms to be filled out in the office by arriving 15 minutes early to your scheduled appointment time. Payment Options
For your convenience, we accept cash, personal checks, money orders, and most major credit cards.
Dental Insurance
For your convenience, we collect all insurance information before your scheduled appointment to ensure that we can verify your coordination of benefits. This will allow us to give you the best treatment estimate on the day of your appointment.
It is common for insurance companies to cover 80% - 100% of dental services, however, orthodontic benefits differ slightly. Insurance companies, in most cases for orthodontics, will cover 50% of the treatment fee up to a specified lifetime maximum, which is different per carrier.
As a courtesy to you, we will be happy to file for orthodontic coverage with your primary carrier. After coverage is verified we will agree to bill and collect from an insurance carrier. However, we ask patients to understand that if coverage ceases or changes, the patient is responsible for the agreed-upon fee. If any or part of the insurance fee is not paid, it does remain the patient’s responsibility. Please be mindful that treatment fees are always an estimate so you need to be familiar with your insurance plan. We are contracted or participate with most PPO insurance companies. If your insurance is not listed below or you have any questions regarding your insurance plan, contact our office.

As a courtesy to patient families, Dr. Nay offers a variety of payment options to meet their needs, including:
